Camper Application Form (If signing up by snail mail or fax 805-927-2610)

Important forms that must be completed after registration. (Health Forms must be received by camp two weeks prior to camp session)

• Health History Form 1 (Must be filled out for all campers yearly)
• COP Permission and Waiver Form

Other informational forms.  These we mail upon registration, but are also available here in case they get lost.

• Parent confidential sheet – notes from parents to camp directors/staff
• Camper info form- notes from students to their counselor
• 2012 Parent Handbook Final- Important Info about Camp! (Changes have been made from the 2011 version)
